End of Day - Collection Routines

This form is for parents to inform the school where they would like us to release children at the end of the day rather than waiting to hand over pupils (to an adult) in the playground. PLEASE NOTE: this form is not needed for pupils in Year 5 and 6 who are dismissed independently from the classroom each day.

 

 

Report a Pupil Absence

Please use this form to report any pupil absences. Please note that reported absences may be followed up with a call from school.

Request to Administer Medication 

If your child is unwell, they should stay at home. If your child is well but has been prescribed medicine by your GP. Please ensure you complete this form if you require school staff to administer this medicine in school. All medicine should be sent in to school in the original packaging, with instructions and means of administration i.e. medicine spoon.
