Online Forms
As a result of us asking parents not to enter the school during the coronavirus pandemic, we have made available many of our usual forms in an online format. Please see below for the form you are after. It is always advisable to contact the school office to let us know you have submitted a form for our attention. End of Day - Collection Routines
This form is for parents to inform the school where they would like us to release children at the end of the day rather than waiting to hand over pupils (to an adult) in the playground. PLEASE NOTE: this form is not needed for pupils in Year 5 and 6 who are dismissed independently from the classroom each day.
Please use this form to report any pupil absences. Please note that reported absences may be followed up with a call from school.
Pupil/Parent Information about our use of video conferencing
Before your child will be allowed to join their class’ first Google Meet, we will require both parent and pupil to read the information in this form and submit to demonstrate you have read and understood the information within it.

Request to Administer Medication
To help reduce mobility around the school and maintain the integrity of our small groups/bubbles we have made a small number of changes to our administration of medicines procedures.
- We will no longer be administering non-prescribed medication - if your child is unwell, keep them at home.
- Our administration of medicines form is now a digital form (click in title link above). Parents must also contact the school via telephone to confirm, in person, their form submission.
- Medicines will be kept in the school office (as usual) but taken to the appropriate classroom to be administered by a supervising adult within the class group.
- The adult may wear PPE (disposable gloves, apron and mask) depending on the symptoms shown by your child).
- Parents may drop off medicines (once agreed with the school) at the school office and collect at the end of the school day. Where medicines are to be used across a week, these can be kept in school if appropriate.
